custom form development using PHP and MYSQL

We are building a new online platform to assist with post-Hurricane Sandy relief effectiveness and transparency. We need this project completed asap. • Update one custom PHP form and create another. for example, the first form looks like this: [login to view URL] and we need to add something similar to this: (styled better) [login to view URL] The other form is shorter. Will send details in DM. • Add corresponding fields (from both forms) to existing template page and style • Connect new form fields to existing MYSQL Database. Tables exist already. • Test and Troubleshoot - everything should be working and connected to database/site. Details available via DM
