Adwords Landing Page (High Conversions required)

We are soon going to launch our new site and we need an excellent landing page for our adwords campaigns. The landing page should be a separate page and not integrated with the current site. Please pm some examples of great landing pages specifically being used for Adwords campaigns right now, that have proven to be high converting. The landing page needs to have all the elements that are required by Google to give a high quality score as well. We will give you the website address via PM once you have shown us your proven track record and examples. Dont send us your portfolio of web design. We are only interested in seeing you Adwords Landing pages, what makes them work so well and the conversion skills you have. Our objective is to either make the phone ring or have them visitor complete a basic form for a sales person to contact them. The form will need some PHP capabilities to email the form data to about 4 different email addresses (basic PHP function). It would also be good for the PHP to use CURL to also add people into an Aweber email list. The actual design can use the same overall design colours for the existing site, but will need engagement and conversion elements to be styled on the landing page (be it photos, call to action with the phone number, call to action with the lead generation form etc).
