Adobe Air Sample App

I need a sample adobe air application that will do the following: 1. on load, read from a database table that tells it what the text of a label should be. 2. form with on screen keyboard and three input objects. 2a. text box with max length 2b. pull down with items from database to choose from 2c. radio buttons to pick one of. 3. on submit of form, save data to database. 4. upload data saved to a webservice and mark as uploaded I will provide the data structures, sample data, and webservice. all source code me be provided at end of project
