comment box created 1 html page

i need 1 html page made with a comment system below for customers to leave comments , let me know how i can link it to another page. i know where to put the link i just need to know where i am supposed to find the link. before the comment box starts i want a screen shot and text in that page. the comment box should only have 1 more field called name, users to manually type in their name. basic . no design required
