I have simple 6 page document that needs to be created into an online fill able form in jotform.com. 1/3 of the fields will be fill able for custom information for the customers name for example. 2/3 of the fields are check boxes. The form would need to be broken into different sections eg not one long page. Maybe broken into 6 pages or what looks best. I like [login to view URL] as the file can be upload into an CRM easily.
