Alert Box Customisation

There is 2 parts to this project:

PART A

I have a form which gives an alert in the following situations:

1. When the form has been submitted successfully; or

2 is one of the fields is empty.

Currently the javascript alert box's text is left aligned and OK button is right aligned.

I need 2 type of customisation:

1. when form is submitted successfully - Text and OK button should be centralised

2. when form has missing fields. - Text to be left align and OK button centraised

PART B

Alert box shows domain name, I would like it to show like other information.

Scripting should be done in javascript.

Please quote both parts separately. If cost is too high, we may just do Part A.
