We have created an online survey for our clients. Example here: (Link will be provided to prospective Freelancers)

This needs to be delivered into a report for the client and emailed to them. The report outlines the answers to their questions and suggests a strategy based on a scoring system within the survey. The report needs to professional and include templated text as well as the clients details entered, then sent to them and us via email. Ideally, we want to have a generic report that can be branded under different company names depending on where the link to the survey sits. ie. if the survey link is on ABC Website then the report is branded as ABC.
