Need a service request form clients can complete and submit to us via email. We want clients to have the option of filling in their name, their firm's name, contact info, etc and saving the template so they do not have to re enter this info each time they use the request form. When we receive the service request form we want to import the client contact info and the specifics of the request into our research report template (which we already have, but which may have to be modified - as part of this project - to handle the importation of data from the service request form. The service request form will be a one-page form.
