5 steps PHP form

We have to build a five steps PHP form with the following specifications:
- client side validation including remote validation ([login to view URL])
- server side validation including remote validation
- CRUD
- populate form based on a session token (if a visitor would want to resume completing the form some other time)
We would prefer:
- developing with Php Form Builder Pro ([login to view URL]) OR
- an alternative mature and modular plugin/framework
- Bootstrap and/or Material Design framework
Basic steps structure:
1. General details (select, input, radio)
- some groups are conditionally shown
- two remote validations
2. Addresses (input, select, radio)
- some groups are conditionally shown
- two address autocomplete (API from Google and/or from outher provider)
- two remote validations (address validation using the above API)
3. Person(s) details (select, input, radio, checkbox, date)
- a person will have name, address, date of birth and place of birth
- some groups are conditionally shown
- one address autocomplete per person (API from Google and/or from outher provider)
- can be one or more person with add more / edit / delete
4. Entitie(s) details (select, input, radio, checkbox, date)
- entity can be person (name, address) or company (name, address, company id)
- some groups are conditionally shown
- one address autocomplete per entity (API from Google and/or from outher provider)
- can be one or more person with add more / edit / delete
5. Connection(s) details (select, input, radio, checkbox)
- select individual items from step 3 or 4 or select a group of items from step 3 and 4
- add few attributes to selection
- sellection and attribute allocation to be valid, some basic math should be checked on two fields
- can be one or more connection with add more / edit / delete
The joub should start asap. We will provide all info required, including validation rules, field specs, conditional logic, etc.
