HI there can you check out my form and get it to send to my email?

1) Do not ask to use contact 7 (it is not the solution because there is a query to a database in order to submit)

2) Query confirms the number entered by a user matches number in the data(like password) before it can submit this is already coded and must continue to work.

3) Must work now not after you sleep so d not bid if you do not have 1 hour to debug form.
