Fix a simple issue on the check out page on [login to view URL]

Hi,
We have issue on the check out page. customers wont be able to submit the order. they asking to check the age under 18. but there is no where to check the button or confirm the age. First please identify the issue and give me details as to what exactly the issue as I understand the code very well .
then I will provide access to FTP to fix the issue. couple of months ago we have changed some fields like first name and last name to full name..on the check out page.

As the issue is intermittent once fixed I need some time to test and wait for customers if any issues. thanks .
