I need an android mobile app which will be used for family survey by our selected users, in this application 1) Login, 2) Registration, 3)Survey, 4) Survey Report - Date Wise, District Wise, Tehsil Wise, 5) Search Members by name and mobile number. these options should be there.
Survey Form Fields are: first name, last name, gotra, father name, mother name, dob, gender, blood group, email id(optional), mobile number, alternate contact number(optional), marital status, spouse name, child, current address, permanent address, same as above option should be there for second address, block, post, village, district.
Photo, Group Family photo.
Mobile number verification option should be there by OTP after successful registration.
Admin panel in PHP mysql.
