PHP fetching incoming emails and doing commands (like procmail)

I am running a web app that sends on a hourly basis information via emails.
I need to process user's replies (email replies) to be able to recognize some special keywords like stop, pause, resume.
On Unix this kind of tasks could be done via fetchmail/procmail, I need call back a PHP script to process these sort of commands.
System is : Debian, IMAP Dovecot + Postix
If it is using procmail or any other external tool I may only need install procedure & a sample code, and we'll do in on the preprod server.
Needed for next friday.
