upload files to folder on my server

I need some one to build me a file upload form for my site where someone can upload a selection of picture to my server. I also need each upload to be in their own unique folder as well, for example I will have a main folder on my server called "/images" then each user will get a sub folder in that that foolder with their images ie. "/images/user1, /images/user2, /images/user3, etc. I will also need the script to email me once a new upload has been made with details of the user ie. name, email etc.
Basically I am offering a slideshow service to real estate agents who will upload their pic to me and i will create a slideshow for them.
