Redesign a PDF form

I have two PDF files which need to be redone. the layout is fine as it is. i need to put a logo on top of each of the files for each form each form is very similar, but the logo at the top needs to be different. additionally the forms need to be filled out electronically. I would like it so these can be filled out and emailed. I have the high res logos I require to be on the forms. The end product will be with new logos and will work with fill-in blanks electronically so they can be emailed with the information in them. the instructions at the top should include instructions to email the filled in form.
