Fixing Wordpress Error Widget

I'm Tuong, i'm designing a wordpress theme but i'm not good in programming.
I attach here the social icons widget file, it allow the bloggers can check the box/the icons that they want to show up. But it always show the error "Notice: Undefined index:" (take a look to my attached image). The errors are maybe dissappear once you save changes for the widget, but they still show up on the php log.
I need a developper that can help me fix the error as soon as possible.
