I need webdesign and implementation of a very simple (one-page ?) service ordering website '[url removed, login to view]' to proof an idea.
The page should be segmented into 4 parts:
1) plan selection
- also table Service x PlanName
- Price
+ possibility to buy additional 1-2 extra-services (ie. express response etc..)
smth. similar to [url removed, login to view] plan selection - [url removed, login to view]
2) Attach one document and add description
- select document type from dropdown
- select document for upload
- additional free-form text description
- email address
3) Disclaimer
- readonly textarea with some legal statements...
- explicit checkbox to agree
4) order summary
- textual order summary
- captcha
- Send button
Notes:
- To make this site respond quickly, I prefer having all this parts on on one page, with avascript doing all the verifications and page dynamics and Send button should send all the data to wordpress at once.
- HTML5 preferred
- mobile support required
- buttons in CSS
- simple solution is preferred
Delivered project should contain:
- simeple logo (in SVG)
- HTML/CSS representation of design
- Wordpress 3.6 templates + Javascript
