More fields in an opt in form

This is a job to simply to alter some code and give me two more fields in an opt in form. The current code is provided in the attachment.

Instructions:

I want the name field to say "Full Name" (required)

As well as name and email fields I want "phone number" (required) and "website" (not required). If there are any questions fire away.
