Code PHP - jQuery Upload Form

I need to have a file upload form created using PHP and jQuery that displays the progress of the file(s) being uploaded. There are plenty of classes or libraries out there that are already developed that you can modify. i.e. [url removed, login to view]
Please see my attached file. The width of the PHP div's must not exceed the width of the graphic as this will be a pop-up form.
When the form is filled out - the should be uploaded to a folder called /submissions
The owner of the form should be sent something like this:
Subject: Custom Pendant Request from $name_field
Body: $name_field submitted a design on $current_date at $current_time.
Here are the files that were submitted: $file_form1, $file_form2, $file_form3
$name_field's Phone Number is: $phone_field
Please see the attached JPG to use as a guide.
Also note that I do not need these fields: Street, Address 2, City, State, Zip Code or Country
The $file_form1, $file_form2, $file_form3 needs to display the actual file-name that was uploaded but provide a direct link for the receiver to download the image(s).
