Application form for an apartment complex:
2 pull-downs: Apartment Type, Apartment Level should pull from database (mySQL would be fine).
Monthly Apartment Rent will calculate based on a formula of the Apartment Type and Level.
Submit form button should have two possibilities. 2 buttons, one for “Pay by check” that directs user to one landing page, and “Pay now with Credit Card” that directs user to payment page which uses paypal.
All fields with * need to be made required fields.
Built using Formtools PHP script with database.
