Change a Joomla Template to new design on Home Page only

The clients needs changes to the design of an existing Responsive Joomla site

The top of the page there is a large slider, with only one slide and the text ""Welcome to Treasure Hunt. Thanks for letting .... etc."
We need to replace that with two columns (1/3 - 2/3 ) instead of one column.
First column (1/3 of width)
- responsive video (embeddded Youtube or Vimeo)
- "Contact Us Now" form with name, email, and description of item. (These must hints inside the actual form fields as there is not a lot of space)
Second column (2/3 of width)
- static image
We will give you logins to to admin, FTP access, vidoe links, graphic files, text etc.
We need you to adapt existing template to match new design and upload all new content.
