Data Input / Sign Up Form Script integrated with spreadsheet

I'm looking for a script to be run locally on my computer via browser which will allow people attending a stall to sign up with their information.
The script should allow them to enter their details via the browser, it will then dump these details into a spreadsheet - csv or xls, post a new page saying "thanks for entering your details" etc for 5 seconds before returning to blank sign up form for next person to sign up.
Ideally I should be able to customise the html of the browser window file to look how I wish.
As an example you can look at these:
