I have an existing site and php based web form. Currently the form (php include) is just on one page, the layout of the page needs to be changed to a 2 column layout (using an existing shortcode) and then dupicated to the other 6 pages.

On one page I would like a background and border adding to the form.

Finally the whole site needs to be transferred to the root (at the moment it is in a sub folder and files are referenced by a htaccess)

It is a fairly simple job but does require someone who is proficient at css and php as the form uses smarty templates.
