Create a word template for scientific articles from a pdf

Contest Brief
I had used indesign to organize scientific articles in the past; however, I found it difficult to use and to share with others, so I want to move to a designed word template where I can just copy and paste the content and make some formatting changes in this platform instead of indesign. I have a pdf document with the design (v4i2a4), and I want to find somebody to design my word template with all the features, including titles, subtitles, header, footnote, their side changes based on even or odd page numbers, the references section with smaller font and in a different page, and the acknowledgment section in the reference page in a limited box at the end of the references. I am attaching two documents for reference: a pdf and a word. The pdf (v4i2a4) is the design that I want to have in word and the word is just an article that I want you to show me that is feasible to prepare in word.
I want (1) the word template, (2) the article copied and pasted in the word document, and (3) the word printed as a pdf as a proof of the accuracy of the designed template.
Most of the fonts used are Delicious (attached). The journal name font is Trajan Pro except for the word "of" that is times new roman.
The authors' affiliation font is Adobe Garamond Pro
Font size and line spacing: Title 20 size, 22 spacing; Authors' names 8.5 size, 10.5 spacing; Main article text: 9 size, 11 spacing; Authors affiliation: 7 size, 9 spacing; Acknowledgement section 8 size, 10 spacing; Subtitles (introduction, methods, results, discussion) size 12, spacing 11; Figures and tables names: size 7, spacing 8.5; References size 7, spacing 8.
The color of the font describing the type of article (in this case "Original Article"), the author names, and first level subtitles is R 26, G 42, B 121.
Please check the file v4i2a4. I want the word to be able to build that pdf!
