A web app for our Insurance Business

The project description is as follows:
The app is to be used by our Insurance sales personnel. A registration page allows each one of these users to be registered into the system. The landing page is a user login page . On login, the user is shown a customer data entry page. This is used to collect the data from the customer like name ( entered manually), age /address etc ( entered manually), the selected health policy ( comes from the database) and the corresponding annual premium amount ( comes from the DB). Submit button to send the data to the master database( saved successfully ).
The system should send a whatsapp and SMS message to the end customer that " the policy request has been received and subject to KYC verification and cheque realization, the policy will be issued within 3 days from the date of request"
MIS table in the system should show a single table with details of all requests received with a search option ( based on name , date , policy details).
It is preferred to run the system like a web app ( sub domain) under our corporate website.
A simple 2 or 3 page website is what is envisaged.
