Update a Google Spreadsheet / Calendar Script

We have a Google script that is suppose to create new Google Calendar events based on information on a Google Spreadsheet (see attached). The script no longer works with Google Calendar, so it needs to be updated. More specifically, it appears that the script executes properly, but Google Calendar only creates the first event (//create event on 5 years) and ignores the other events (for example, //create event on 6 years ). To be considered for this project, candidates should include their work samples of past Google Scripts. Thank you.
