time tracking tool (mobile web app)

I need a basic time tracking app for our team to check attendance, time off, sick leave, and so on. this needs to work on mobile with a login functionality for every user. role concept for employees, team lead, manager, super admin with approval loops. base config of the tools needs to be editable (min hours per day, bank holidays per team/location, working days and off days, and so on) simple button to check in and check out for logged in users times can be changed but changes need to be approved by superior paid leave and other absences can be requested but need to be approved. monthly reports to each user that need to be sent in for approval, superior can approve months for each of their eployees. every employee can see their current hours saldo and number of paid days off left (including planned) feel free to ask any questions on this project. desired time line is design done within 1 week (incl. review and up to 2 cycles of feedback/changes) and after that 1 week of coding and testing. please indicate having read and understood all requirements above by starting your bid with * needs to be coded in PHP 7+ and MySQL frontend to be coded in html5, css3, javascript (free to use libraries like jquery) frontend design needs to be original, modern, clean, and approached mobile first ownership off all designs and code end up with me hosting will be provided milestones will be created for design, coding, testing and will be released on finalisation of each phase.
