simple wordpress plugin

Before you bid, read carefuly I require a wordpress plugin that allows the admin to create a multi step questionnaire, the questionnaire is a sequence of steps, each step contains one question and multiple answers, basically the same step repeating itself over and over (with a different question and different answers) until it reaches the end. as you may have guessed, I want the admin to have a user-friendly panel (within wordpress) that will allow him to create questionnaires, in each questionnaire he can add questions(steps), and for each question as many answers he wants. additional requirements: -each answer should have a value, a text and an image -each questionnaire can be called through a shortcode let's say [questionnaire id="123"][/questionnaire] -the generated questionnaire, should send an email containing the answer to each question to a specific email I am not looking for a regular developer, I believe any wordpress developer can finish up this project easily, I am looking for someone who could provide a flawless result.
