I NEED AN "ONLINE AND OFFLINE SURVEY MAKER, PREFERABLY IF IT'S ALREADY DONE

I'm looking for a Survey Maker or Web Form Maker that works ONLINE or OFFLINE, that has functions similar to this tool online but not so sophisticated: [login to view URL] IMPORTANT: Survey Maker could work ONLINE on a server, but Surveys must be able to be applied OFFLINE. If you have a previous work or an already made script with this functions (see below) I need it, maybe I will have just to change the styles and layout but same front and backend. ---------------------------- Survey System Features These are the most important functions of the Survey System: 1. Should work on a web platform whether online or offline (very important) 2. User should be able to keep the results in the device’s memory, this way the surveys could be filled even when the device is offline. 3. Formats for the input data made by the Survey answers should be in CSV or XLS or any format compatible with MS Office. 4. Input data made by the Survey answers should be able to be transferred into a USB device or sent to a specific email assigned in the admin config. 5. User should be able to create, edit and preview every survey he creates; these surveys will be listed inside the backend. 6. The Survey System should have a login access request in the backend for the administrator. 7. The Survey System should be able to create Surveys in a responsive HTML/CSS format. 8. A nice, clean and solid user interface is needed. I need this within the next 2 days whether you already have it or you can make it, better if you have a previous script just to make it work in a server end.
