Interactive Map and Contact Form Linked to Google Sheets

I am looking to create a website which has the following pages and features: -Age Verification - Interactive map (like this: [login to view URL]) - Contact form (like this: [login to view URL]) - Information form 1(Similar to Contact form style, responses will be linked to google sheets) - Information form 2 (Same as Form 1, only few different questions) - T & C page - Privacy page Age Verification: That is pretty straight forward. When logged on the website, the user will be requested to enter their birthday to make sure they are above a certain age. Interactive map: The website I shared uses [login to view URL] to upload image of a map, but you do not have to use the same. It is important to use a stylized map like snazzy maps or similar open source. There needs to be a backend to add image, location, and content similar to the website sample I shared. It will be updated frequently. Contact Form: Basic contact form, it will have the same design as this template: [login to view URL] I will get email notification when someone submits on the contact form. Information form 1 & 2: These 2 forms are the same, only 2 questions will be different. The important part is to link the answers to a Google Sheets document. Since the form is divided into multiple steps, each time user hits on "Continue" button, the information will be automatically filled in the Google Sheets. So if a user stops half way during filling out the form, we will still have some of the answers. T&C page: This is a basic text page. It would be good to be able to update from backend as well Privacy: Same as T&C page You will be provided will the designs in PSD file. It is important to execute the design and animations without glitches and load times. Of course it needs to be responsive. You are more than welcome to use the template links I shared with you here to save time. But if you make it slow and buggy and blame the template, then please don't use it. If you are able to follow rules of progress web app to make everything load extremely smooth and function well, that would wonderful.
