Incorporate Simple Payment Form Using Square merchant ([login to view URL]) PHP

I need a PHP script to be able to include in other pages that will create a simple payment form for the user, populate a payment amount and return information regarding if the payment went through or not.
To simplify:
1. Create a simple payment form using Square merchant ([login to view URL]) , populated with given variables (stored in sessions or current PHP script) with the price to be paid and customer info. Use their API or anything else they offer.
2. The user fills out the information clicks pay then any information returned from the merchant should be stored in an associative array which will then be populated in a database. This job entails only the payment form and providing response data from merchant, I will handle the rest.
