Create PHP sample code for file uploads to [login to view URL] OCR API

I am looking for a PHP sample code that allows me to upload PDF files to the free OCR API. You can generate a free api key for testing at [login to view URL] (the free api keys allows you to ocr the first three pages of a PDF document).
The website itself can be very basic: It should take a PDF file as input ("Browse" button to select a file) and then display the OCR result from API (status code and OCR'ed text). I tried with the code at [login to view URL] but it does not work for me. I am a PHP beginner. So this should be a quick project for an expert.
