Convert PDF to Word and Create Google Docs Web Form to Populate Fields

We currently have a form in PDF format that needs to be edited in Word. Once the form is edited, we need to create a web form using Google Docs where we can populate the form and have the saved PDF pushed to our MyCase Case Management System ([url removed, login to view]) where the document can be associated with a My Case file and stored within the documents folder for that file.
We anticipate 5-6 hours of programming needed for an experienced programmer.
