Build a disclaimer and agreement page in Wordpress

I need to have a wordpress page built that has the following features:
1. Visitor wants to visit a specific page in wordpress.
2. When visitor enters that page a disclaimer should be shown, where visitor needs to choose which country he lives in and which country he is in right now.
3. If any of these options are some specific countries i.e USA, Australia or Japan, a text should be shown saying its not permitted and he should be sent back to startpage.
4. If any of the other countries are chosen, a longer disclaimer should be shown. And also 2 radiobuttons where you need to accept the agreement. After accepting the agreement the real wordpress page should be shown.
I will send more details and a sample of how it should look in private message for those interested.
Access to the real site is not permitted, so any development/build should be made on your own server and then send a package with instructions on how to install/enable it.
