Form Submission Through XML & Add Form Fields

Modify a form on our page so that when a lead is submitted the form will ping through XML a website who will ping back to us a response and if successful we will post all the entire set of information to them.
1. The form fields need to be mapped to a XML schema. We will provide you with the XML schema. You will need to know how to pass variables from the form and to be able to collect data that gets posted back.
2. The form has some existing fields already, but we need to add additional fields and map these fields to the XML schema as well.
3. Clean up the responsive CSS so the entire form and all the fields display appropriately on desktop and mobile.
You must be able to communicate in English and will need to be able to chat during business hours 6am - 11:59pm Pacific Time (US & Canada)
