Custom php and sql script

I need a simple php sql script.
For a profesionist this would not be a problem.
It is a customer manager platform, where I can keep track of customers.
- I need to be able to add custom fields and each field should have the tags like [display_name]
- The system will send automatic reminder emails to customers on X days before expiration (via cron I think)
- I can create as many custom fields I want, and as many email reminders I want.
- Each customer will have a button where I can prolong customer's subscription with XXX monts/days/years.
- The tool must come with an installer.
That's pretty much it.
I don't mind if it is based on platforms or frameworks as long sa they dont get errors and it does what I need.
I am not trying to be rude, but please indians don't bid.
And please indians DO NOT ASK me: "please explain your project.."
