Custom WordPress Questionnaire Plugin

I need a custom WordPress form plugin that will allow the following: SUMMARY It needs to handle numerous questions (hundreds). Questions need to be grouped into sections that can score the totals. It needs to scored numerous different answers. It needs to save the results for the user. Questions will consist of multiple choice or fill in a text area. Only multiple choice answers will be scored. Fill in the text area answers will be passed through and saved but not scored. The questionnaire needs to be able to display on a page. The results need to be able to display on a page. For example: Section One: Sleep Habits Question One: How well do you sleep at night? Answer One: Not Good. (1) Answer Two: Somewhat Good. (2) Answer Three: Good. (3) Answer Four: Pretty Good. (4) Answer Five: Very Good. (5) *The point values should not be visible to the user. There can be an unlimited number of questions in each section. Each section should be able to score the results, for instance: 1-20 (Description of score), 21-40 (Description of score), 41-60 (Description of score) and so on. And in the results page, it should show the score of the user for each section with the corresponding description. So I need to be able to have multiple sections, multiple questions, and multiple answer with customizable values assigned to the answers. Ideally, the results are saved and can be displayed on a front end page.
