Code and develop a form with inputted parameteres directing users to specific webpages

Hey guys! I need a basic form with username and password and button created for users to use as a login to a dashboard. I need the form coded so that when a specific username email address is inputted and password with that email address, it directs that user to a specific webpage (already developed and hosted) - and to have the code easily interpreeted and used by me for future use when I get new clients on board, so I can easily edit the code and add in new email address's and passwords for those new clients. - Develop and code form ( to fit in with design of webpage - link can be provided) with username and pass word fields and a button. - Have specific inputted parameters direct that user to a specific page that is different for other parameters that are inputted (that have their own specific different webpages). (Multiple parameter directing to webpage able). - To have the code for use by myself for when other clients join, for me to add their email address and password to the code, and update it on my website to have their credentials direct them to their specific webpage.
