Add a table to Wordpress displayed though widget

I need someone to add a table to my website which is displayed on the submit form page, where users can enter data then furthermore i need it to be able to displayed on the listing page. You would need to create a couple wp job manager templates. The table you'll be copying is basically the same as the one displayed on hours of operation but the information will be different, view the images below Form [login to view URL] Listing page [login to view URL] I've attached the files which currently display the current table in which you'll be duplicating to show other fields such as "Rates" Incall Outcall 1 hour 2 hour 3 hour 5 hour All Night Please don't bid on the project if you have no idea on how to do this, it's not simple and you can't just install a custom made widget to do it either I look forward to working with you.
