Online race registration/database - reposted

Hi, This is a repeat posting for this job as the previous programmer has literally disappear without a word. I need someone RELIABLE!!! I need to rebuild my online race management system from scratch. The current one was programmed by many different programmers over the years and the coding is a mess (I've been told) so there have been problems. There are three sections to the my online race management system but this is just for the first part - online registration. I've attached a basic flow chart of what I need. I've worked with many programmers through GAF and most have been very good, but I'm finding that they tend to disappear or not respond to emails when I need more work done. So I need someone who is reliable and professional. I am NOT INTERESTED in working with a large company but an individual, so don't waste your time or mine if you are a company of programmers. I want to develop a long-term working relationship with ONE programmer. I do my own website design using Dreamweaver, and utilize Dreamweaver templates, so your php programming MUST be compatible with this so I can make graphic and text changes. Your English must be good so that communication is easy - having a Skype account so we can talk is a plus.
