GIS/Location Mapping

I have 5900 names on a mailing list. I want to provide a map from the consumer address to the closest business location that can be printed in high resolution on a 600 dpi printer. All graphics will need to use the same scale and radius logic (i.e. finished maps to be no larger than 3" w x 2" h based on a 10 mile radius). I have all the associated data (closest locations) and zip codes of the providers (there are 4360 possible matches). What I will need back is 5,900 printable graphics tied to specific list names in an excel formatted file. I do NOT have latitude/longitude data on any addresses. The business location (the closest, only) will need to be indicated with a star or similar graphic. I am attaching a sample web graphic that shows what I mean - I will need high resolution graphics approximately 3" w x 2" high for the finished project. I have also included a small test file of 10 names for you to complete and provide back with your bid. I will not award the project without seeing that you understand the basic project parameters and can provide high resolution images from either Google Maps or Microsoft Mappoint.
