I need to develop an application that would allow a user to search the database and allow comments to be posted. The data for the database can be located at: <[login to view URL]> What I am looking is something to a front end similar located at: <[login to view URL]> On the details page located at: <[login to view URL]> I am needing an option where a person can enter comments about a particular drug. The comments would be displayed on the details page as well. For the customer to add a comment there must some kind of form for the customer to add from (Most likely on the details page as well). This form should include several fields like name, comment, and so on. The form should also have capcha to prevent spam. In addition the program should have a admin section so that the customer comments could be edited. In the database there should be a table that includes customer comment information, date of entry, and associated commenter's information. In addition the program should be designed such that it would use search engine friendly URLs similar to a mod_rewrite on Apache. In the bid please state what components and 3rd party programs would be needed to complete the project as stated above. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
