online contact form writing details to linux file

Hi,
I have a mobile website page that I would like a contact page added to, it needs to show name, email, telephone and comment with name and email being the compulsory fields. When you hit submit it goes to another page that I will provide.
The data I would then like to write to a file on the linux unit which is located at var/log/messages
It should be written like this
date : time : html page name : name : email : telephone : comment
