App to read hospital labels, OCR them and return text

Hospitals print pages of standard stickers for use on various documents. For any given hospital these stickers are formatted in a constant manner and display a fixed set of useful fields of data.
As a proof-of-concept piece I need an app which will allow the user of an iPhone or iPad to photograph an image of a hospital sticker. The image may be processed in device or on a remote Linux server. An open source OCR solution should be employed (eg tesseract-ocr) and the key fields should be returned as text to the phone. A simple results screen with the text results is all I need at this point.
I will provide an example image. The solution should identify, at a minimum, surname, title, first forename, date of birth, and address (as a single line of text). The solution should be able to get this data from any reasonable image of a sticker from the same hospital. The solution should provide scope for generalisation to other hospitals. The user could select a hospital name from a drop down list if having a template makes the process of field recognition and ocr easier.
