Project proposal: Create a HIPAA compliant Ipad app that allows an office to fill out an intake form that is sent from the Ipad to our secure website. A copy of the filled out intake form would also be sent to the same office computer for archiving. App locked so that it cannot be used outside of our specific system – proprietary use and owned by me. Open to simplified design of new secure site that will work easier with new intake form use. 1.) Medical intake form filled out on Ipad screen by technician. Program designed so that if any area is not filled out the program will not allow the technician to go further. 2.) Signature area for patient to verify accuracy of filled out form. Once done tech clicks SEND. Program resets with new blank form ready to be filled out. 3.) Intake form saved with patient name, techs last name, and date (Smith, Jane-Jones-2017-9-17. PDF). 4.) Copy of intake form sent to office computer for filing. Ability to select file on office computer to where form will be sent. 5.) Copy of intake form sent to our secure site for downloading to our computers for archiving (Batch or office specific?). Access to our secure site via unique username and password. 6.) Data on intake form extracted and placed on our MS Word report form with patient name, techs last name, and date in MS Word format (Smith, Jane-Jones-2017-9-17. doc). 7.) Report form sent to forms area in each office’s area on secure site. I go to secure site and download intake-ready form for me to use.
