We have a wordpress site. [login to view URL] We want to create a cancellation form that is a hidden link on our site. This form can be sent to customers who wish to cancel their services. Once they complete the form a copy should go to our accounts team and also the customer with an attached PDF. The attached form is basically what we want. The fields are all good. We would replance Brennan IT with the name Telegate. I imagine that any form you create we can edit the text easily through wordpress ourselves so this can be done later.
