Build an online Business Questionnaire

I want an online assessment built. I want people to be sent to a page where they will complete a series of questions. The questions will be answered as a score between 1 to 10. There will be at least 10 questions. Once they have answered the questions they are to be sent to a page were they will see their results. These will be in two forms. First a graph of the scores on a wheel ( a circular graph - outer circle is a score of ten, there answers shown as a point and each point connected). The second way will be a description of scores. Eg between 0 and 50 with a description; 50 and 75 with a description etc On the results page I need an option for people to register for a more detailed assessment ## Deliverables Provide an initial concept design. Professional design and complement the current website design. Be able to report via skype or phone. Provide weekly updates on progress. Achieve agreed upon timelines.
