Auto Sort files into folder

I am a photographer who takes many photos of many kids. Before we photograph a child, we take a photo of a sheet that has the child's information on it - including a barcode of the folder number. Currently we manually sort the files into folders after we download the files. Would like software to read the barcode on the camera raw file, create a folder named the barcode number, then sort the following files into the folder. When the software sees the next barcode on the raw file, do the same- read number, create folder, put all files that follow into the folder.
Additional feature: Would also like the program to be able to change the file names to the barcode number + serial number and be able to imbed the barcode number and possibly other information into the meta data of the file.

Operating System: Windows
What kind of camera(s) are saving the RAW files? Currently using Canon 5D Mark II, and Nikon SLR but would like the software to handle a variety of camera raw files. If we had to use a JPG, we could take a photo of the barcode as a JPG.
