Air Data collection

I need a desktop Adobe AIR application, which will allow user to fill Automated Form and submit it to his manager for approval. Manager should review the items, which user added to the form and see the consolidated data results and generate excel export with the result. User must be able to add new lines in the form and choose if he chose the item from the list, or if he enters his own data. Each field for each item must have 6 fields and 1 for notes (to describe the reason) All communications between software should be done through server-side webservices based on PHP (need to implement as well) Users must be able to gather within groups and for each group shall be a manager. Groups must be able to be gathered in top-groups with top-managers with same approval functions etc. Everything should be done from scratch. no special pre-installation done. software package should be redistributable, so i must be able to run installations on any apache server and it should connect to give MySQL, create all necessary DB entries and setup PHP data files etc. same with AIR client. should deploy itself.
