I am look for great GUI design. The application will have 3 parts and will be a very small application. 1) Credentialing. 2) Physician Demographic. 3) Income Tracking. Credentialing: Keep track of all schools, organization, Professional Association the physician belong to. It will also keep track of continuing education credits the physician need to keep certification or credential. Physician Demographic: A list of physician and what are there specialties and budget center. Incoming Tracking: This system will tie into the accounting system. Create one table to load accounting information to if present. The system will show how much money the physician is brings into the organization. The system will also show how many patient returns with the same diagnosis code with a 30 day period of time. A process will have to be created to see if second visit was paid for or not. If the physician is having to many of the repeat patients not bill for, that physician is costing the organization money as must be reported as an expense. This information will need to be giving on a monthly basis with a 12-month history and a yearly total. It will be a rolling yearly calendar. Cording Guidelines. 1) A single main form must be created whew all other form will be access form. 2) A single actionlist or actionmanage component will control the entire application. 3) About 5 reports will be created in this application using RAVE visual designer to show Credentialing, Physician Demographic and many on income tracking. 4) All code must be in a double try/except block. Function are prefer over procedure. 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
